Pursuant to Art. 13 of the Regulation of the European Parliament and of the Council (EU) 2016/679 of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (Journal of Laws of the EU 2016) (General Data Protection Regulation) – hereinafter: GDPR, we hereby inform that:

Your personal data is processed in order to respond to the inquiry addressed to Caritas of the Zamojsko-Lubaczowska Diocese in the content of the contact form in question based on the prior consent referred to in Art. 6 sec. 1 lit. a / and Article 9 (2). lit. a GDPR, as well as pursuant to art. 6 sec. 1 letter b of the GDPR, if the inquiry concerns the purpose of the possible conclusion of the contract and pursuant to Article 6 par. 1 lit. f / and Article 9 (2). lit. f GDPR in order to keep records of incoming / outgoing correspondence, archiving in accordance with generally applicable law, supervision over the circulation of correspondence, and in order to possibly establish or pursue claims or defend against claims. In the event that national, EU or international law will oblige us to retain data, the basis for processing will also be Article 6 sec. 1 lit. c GDPR.

4. Caritas Diecezja Zamojsko-Lubaczowskiej may process personal data, in particular data of the following categories: basic identification data such as name, surname, identification data granted by public authorities, electronic identification data, information on financial resources, data on liabilities and expenses, data on financial transactions, details of the person, data on immigrant / refugee status, data on addictions, psychological data, data on marriage or any other form of relationship, data on other family members or household members, data on residence, data on physical and mental health, data on convalescence, data on education, data on profession and employment, data on salary, data on racial or ethnic origin, image and any other data contained in the content of this contact form.

5. The recipients of the data provided by you may be:

Administrator’s staff;
companies providing hosting and IT services to Caritas Diocese Zamojsko-Lubaczowska on the basis of an entrustment agreement;
entities / persons participating in or acting on our orders under an entrustment agreement;
public bodies or entities authorized to obtain data on the basis of applicable law, e.g. courts, law enforcement authorities or state institutions, when they make a request, based on an appropriate legal basis;
at the same time, your data may be transferred to external law offices providing services to the Administrator.

6. We do not transfer your data outside the EU or the European Economic Area.

7. The period of processing your personal data depends on the purpose for which the data is processed and is calculated on the basis of the following criteria:

the period necessary to answer the inquiry addressed to Caritas of the Zamojsko-Lubaczowska Diocese in the content of the contact form;
the period resulting from the archiving obligation on the basis of generally applicable provisions of law;
the duration of the contract, if the inquiry concerns the purpose of the possible conclusion of the contract;
until you withdraw your consent. Withdrawal of consent does not affect the lawfulness of the processing which was carried out on the basis of consent before its withdrawal;
legal provisions that may oblige us to process data for a certain period of time;
the period that is necessary to defend our interests, including the limitation of your possible claims against the controller.

8. In connection with the processing of personal data, you have the following rights:

the right to request the administrator to access your personal data and receive a copy thereof;
the right to withdraw consent to the processing of your personal data;
the right to rectify (correct) your personal data;
the right to delete your personal data, in particular if you withdraw your consent to processing, when there is no other legal basis for processing;
the right to limit the processing of your personal data;
the right to object (for reasons related to your particular situation) to the processing your personal data, if the only basis for data processing is the legitimate interest of the Administrator and / or the performance of a task carried out in the public interest;
the right to transfer your personal data;
the right to lodge a complaint with the supervisory body in the form of the President of the Personal Data Protection Office.

9. Providing personal data and consenting to their processing is voluntary, however, the refusal to provide this data / consent will result in the refusal to respond to the inquiry addressed to Caritas of the Zamojsko-Lubaczowska Diocese in the content of the contact form. At the same time, Caritas of the Zamojsko-Lubaczowska Diocese informs you that it can profile your personal data, however, decisions regarding an individual, related to this processing, will not be made without human participation.
